How Long Does Antidepressant Withdrawal Last? What the Evidence Actually Shows

This is the question everyone asks first, and it is the one with the least satisfying answer. The honest version: it varies enormously, the older clinical guidance substantially understated it, and the biggest factor is one you partly control.

Discontinuation symptoms typically begin within days of a dose reduction, are most intense in the first one to two weeks, and ease from there — but a meaningful minority experience symptoms lasting considerably longer, particularly after long-term use. Meta-analyses put the overall incidence at roughly one in three to one in two people stopping antidepressants, with severe symptoms in a much smaller group. The strongest factor you influence is how quickly you reduce.[1][2]

What the numbers say

Antidepressant withdrawal was for years described in clinical guidance as mild and self-limiting, typically resolving within a week or two. That characterisation has been substantially revised.

Systematic reviews now estimate overall incidence in the range of 33% to 56%, with a pooled figure around 43% across available studies.[1][2] Severe symptoms occurred in roughly one in 30 people in one meta-analysis that excluded patients with confounding physical conditions.[1]

Antidepressant discontinuation syndrome is now recognised in the DSM-5-TR, and is increasingly described in the literature as capable of causing substantial functional impairment rather than being uniformly brief and minor.

Two readings of that, both true: most people stopping an antidepressant do not experience severe withdrawal, and the experience of those who do is real, documented, and was underestimated for a long time.

Short-half-life drugs produce faster and more intense discontinuation symptoms; fluoxetine's long half-life effectively provides a gentle self-taper between doses, which is why it is sometimes used as a bridge.

Duration of use matters

How long you took the medication appears to influence incidence, severity and possibly duration of withdrawal effects.[1] Someone stopping after eight weeks is in a different situation from someone stopping after eight years, and it is reasonable to plan a slower taper in the second case.

This is worth raising explicitly with your prescriber, because standard tapering advice is often calibrated to short-term use.

Windows and waves

As withdrawal progresses, symptoms usually stop being constant and start being intermittent: stretches of feeling close to normal, interrupted by waves of returning symptoms that can arrive without an obvious trigger.

Waves feel like evidence that you are going backwards. They are more usefully read as a normal feature of an uneven recovery. Over time the windows generally lengthen.

This pattern is also why day-to-day tracking is worth the thirty seconds: inside a wave, it is genuinely impossible to perceive a trend. A written record is the only thing that reliably shows you the difference between "this week is worse than last week" and "this week feels worse right now."

The factor you control

Taper speed.

The evidence consistently points the same direction: gradual, proportional reductions produce milder symptoms than abrupt cessation or large fixed-dose drops.[3] The mechanism is straightforward — a slow taper spreads the adjustment across many small steps instead of concentrating it into one large one.

There is a counterintuitive consequence worth stating plainly: a slower taper often means a shorter total experience of feeling unwell. A fast taper that has to be abandoned and restarted costs more time and more discomfort than a gradual one completed once.

Is it withdrawal or is my depression coming back?

The central difficulty, and the one that most warrants your prescriber's input rather than a website's.

Broad distinguishing patterns:

  • Withdrawal tends to appear within days of a dose change, frequently includes physical features — dizziness, brain zaps, nausea, flu-like heaviness — that a depressive episode usually does not, and typically settles if you hold at a stable dose.
  • Relapse tends to build more gradually over weeks and resembles how your original episode felt.

If you are unsure, holding at your current dose instead of continuing down gives you useful information: withdrawal generally settles at a stable dose, relapse generally does not. But that is a plan to make with your prescriber.

When to seek help promptly

  • Symptoms that are severe, or that escalate rather than settle at a stable dose
  • Any thoughts of suicide or self-harm — stop and seek help immediately rather than continuing a taper (US: call or text 988; UK: 116 123)
  • Neurological symptoms that are not typical discontinuation effects: weakness, vision loss, difficulty speaking, fainting
  • A returning depressive or anxiety episode that is affecting your ability to function

Needing to pause, hold, or reverse a taper is not a failure. It is information.
